Epilepsy Foundation Scholarship
This year there is one $1000 award available; however, the Foundation reserves the right to determine each year the number of scholarships to be awarded based on the quality of the applications. This award is not automatically renewed; you must submit a new application each year that you wish to be considered for the award.
PURPOSE:
To assist an individual who has epilepsy with his/her academic and/or vocational training.
MINIMUM APPLICANT QUALIFICATIONS:
Applicant must:
- Must have a current, primary diagnosis of epilepsy/seizure disorder; provide name of physician and name of anticonvulsant medication(s).
- Provide proof of acceptance to a post-secondary academic or vocational program.
- Be a full-time student.
- Submit a completed application no later than November 4, 2011.
- Be a legal resident of the United States, as well as a legal resident of Western Wisconsin. See the attached sheet for a complete coverage county listing.
Note: Awards will be based on the quality of applications through a competitive scoring system that balances need with achievement. The Epilepsy Foundation reserves the right to determine each year the number of scholarships to be awarded based on the quality of the applications. Awards, once given, are not automatically renewed. Award recipient(s) must submit SSN upon request.

April 4, 2011: Start of Application Acceptance Period
November 4, 2011: Application Deadline
November 18, 2011: Winners Award Announcement
December 6, 2011: Award Presentation
